Administration of Medications in School

Whenever possible, medications and treatments should be administered before and after school. However there are times medications and treatments are needed throughout the school day to maintain optimum health for our students. In this event, the school will provide trained personnel consistent with ORC 3313.713 to administer as needed.

In the event a student requires medication, prescription or over-the-counter during the school day, the following procedure must be followed:

1. A Medication Administration Form must be completed for ALL medications. Over-the-counter medications (including Tylenol, Motrin, Neosporin, throat lozenges, etc) must have a Non-Prescription Form completed with parental permission and signature. All prescription medications must have a Prescription Form completed with both prescribing physician and parental signature. 
2. The form and medication must be brought to the school by the parent/guardian in the original container. Never send medication with a student.
3. All medication is kept in the locked storage in the clinic. No medication is allowed in lockers or bookbags.
4. It is the responsibility of the parent/guardian to instruct the student to come to the clinic at the appropriate time for medications. 
5. It is the responsibility of the parent/guardian to alert the school to changes in medication orders and submitting new forms with each change.
6. Any unused medication may be picked up at the clinic the last day of the school year. Any unclaimed medication will be disposed of after the last day of school.
7. A new medication form must be submitted each school year.

Administration of Treatments
Students may require treatments during the school day such as dressing changes, tube feedings, catheterization, etc. The prescribing physician and parent must complete the Health Service Request Form. This form must accompany all supplies needed for treatment.
